Important caveats

BMI isn't the whole story.

BMI is a useful starting point — but it's a blunt instrument. It doesn't measure body composition, where you carry weight, or your overall metabolic health. A few things BMI gets wrong:

  • Muscle mass — Athletes and people with high muscle mass often have BMIs in the "overweight" range despite low body fat.
  • Body composition — Two people with the same BMI can have very different ratios of muscle to fat.
  • Age — Older adults naturally lose muscle and gain fat — same BMI, different health implications.
  • Ethnicity — People of South Asian descent often face metabolic risks at lower BMIs than Caucasian populations.
  • Fat distribution — Visceral fat (around organs) carries far higher metabolic risk than subcutaneous fat — BMI sees neither.

Is BMI an accurate measure of health? +
BMI is a useful screening tool but not a complete picture of health. It doesn't measure body composition, muscle mass, or where fat is distributed. Doctors use BMI as a starting point alongside blood pressure, blood sugar, cholesterol, waist circumference, and your overall medical history.
My BMI is 26 — can I still get treatment? +
BMI 26 is below the typical threshold for prescription GLP-1 medication (which requires 27+ with conditions, or 30+). Orlistat may be available from BMI 28. What counts as a "weight-related condition"? +
For the BMI 27+ eligibility threshold, weight-related conditions include: type 2 diabetes, pre-diabetes, hypertension (high blood pressure), dyslipidaemia (high cholesterol/triglycerides), obstructive sleep apnoea, non-alcoholic fatty liver disease (NAFLD), and established cardiovascular disease. Your doctor will assess what applies to you.
How is BMI calculated? +
BMI = weight (kg) divided by height (m) squared. For example, someone weighing 80 kg with a height of 1.75 m has a BMI of 80 ÷ (1.75 × 1.75) = 26.1. Use our calculator above for an automatic result.
I'm muscular — my BMI says I'm overweight. Does it matter? +
BMI doesn't distinguish between muscle and fat. If you're highly muscular (e.g., regular weight training, sport at a competitive level), your BMI may overestimate your body fat. Our doctors look at your full body composition and health markers — not just BMI — when assessing eligibility.
Does Dutch healthcare use the same BMI thresholds? +
Yes. Dutch clinical guidance follows the same internationally recognised BMI thresholds: 18.5–24.9 healthy, 25–29.9 overweight, 30+ obese. The prescription thresholds (27+ with conditions, 30+) also align with European Medicines Agency (EMA) and Dutch CBG-MEB licensing.
